Getting divorced is a qualifying event that allows you to make changes to your Farm Credit Foundations health care plan. To make changes to your health care plan:

1. Complete the Status Change Online Form.
2. Once your benefit elections have been submitted and reviewed you will receive an email notification.
3. Be sure to review your Benefit Summary which is available on Ceridian Self-Service.
4. Check over your pay advice to confirm your benefit deductions.
