Coverage Options
You may apply for child coverage in amounts of $5,000, $10,000, $15,000, $20,000 or $25,000.  Select one coverage amount and one premium for all children between ages 14 days to age 19, or age 25 if a full-time student.

Current Employees
Within 31 days of the adoption or birth of the child, you can elect any amount of child coverage without providing proof of good health by making the election on the Status Change form.

New Hires
You may apply for child coverage during the 45 day enrollment period without providing proof of good health by making the election on the New Hire Enrollment form.
